Disease Management

Over the past several years IHCS has become actively involved with the Chronic Disease Section of the Michigan Department of Community Health (MDCH). Areas of focus include asthma, diabetes, and injury prevention. IHCS also provides technical assistance to Michigan Medicaid managed care plans in the areas of disease management program design, development and implementation.
